Pre-Employment Requirements
- Valid Drivers License
- Current CPR
- Level 2 background screening
- Two Professional References
- Auto Insurance Card
- Proof of residency or Citizenship
- Professional Liability Insurance
- Physical Exam: completed within the LAST 6 Months, stating that the applicant is free of all communicable diseases
- Social Security Card
- Covid vaccine card
Requirements:
- Complete CEU & Domestic violence, OSHA, HIV/Aids, CPR, Alzheimer
- Certification, and a Health statement showing & free from communicable disease and negative TB test prior to application for employment.
